I will schedule to be available in downtown St. Albert [North West of Edmonton] ever Wednesday afternoon at 3:00 PM Mountain Time subject to at least one person having made a prior appointment. You must confirm that you meet the requirements I have outlined below. Please state which two items of photo ID you will produce before me. One of them must be the document you used to obtain your Thawte ID.
I need the following personal information in your message:
- Thawte ID, required for me to have the paper work ready
- Full name as it appears on your 2 items of photo ID, so I may verify it is consistent with the style in which you have recorded your name with Thawte
- Date of birth to indicate if you are below the legal age of consent
- Phone number in case I need to speak with you
My requirements are as follows:
- Please send me a note for an appointment, confirming you meet my notarisation requirements and include the information above. If you are below the legal age of consent, please confirm you have parental consent to give out the personal information and meet with me.
- Please be there at the appointed time.
- I must see the national identity document you used to obtain your Thawte ID. It must have a photograph. The photograph must look like you.
- I must also see one more government issued or bank issued unexpired, unrevoked identity document which must have a photograph and the photograph must look like you. Either of
- a current, valid passport or travel document in case the primary identity document you used to obtain your Thawte ID is other than your passport.
- a Canadian citizenship card or permanent resident card.
- a photo card [debit, credit or charge card] issued by a major bank. The photograph should be of the front view of your face to qualify.
- a Canadian driver’s license.
- a national identity card for other countries.
Note: Documents which facilitate easy tampering, such as those with a loosely attached photograph, are not acceptable. I have reason to suspect the process involved in the issue of driver’s licences in some countries, which is why I will not accept a driver’s licence issued outside North America as a valid form of ID.
- Your name shown in the photo IDs must match exactly your name as specified to Thawte. The values for the given name and surname must not be transposed. If your name is not written as {Given Name} {Surname}, at least one of your photo IDs must specify which one is your given name [first name] and which one is your surname [last name].
- You must bring a legible photocopy of each of your 2 photo IDs and one colour photograph [no smaller than passport size, no larger than 8″ × 12″, the front of your face shown clearly]. A business card is optional. You must sign the back of the photograph with your full signature and put down your name and date of birth below that.
- I will supply a form that you must fill out and sign in my presence.
- If you are below the legal age of consent, I will assume that both your parent or legal guardian and you understand what a digital certificate is, its intended purpose and what the notarisation in question is about, and that it entails disclosing personal information and providing documentation that might be considered confidential and construed to be a breach of privacy. If I see you it would be in good faith that you have obtained the permission of a parent or legal guardian to see me for the aforesaid purpose.
I will then verify the details, retain the 2 photocopies, the photograph and the form and assert your identity to Thawte.
